Case Management
At The PLACE, every young person has a story, a voice, and a future worth investing in. That’s why our approach to case management goes beyond checklists or referrals. We walk alongside each youth to build a personalized plan that reflects their goals and surrounds them with the right supports to thrive. The PLACE’s primary strategy is to intervene in the lives of homeless and runaway youth and address the neglect, abuse, and trauma they endured to prevent further harm, lessen their long-term risks, and reduce their overall likelihood of becoming chronically homeless adults. We strive for the most flexible and individualized approach to help youth access housing, finish their education, acquire life skills, and build strong, appropriate social supports that will empower them to maximize their health and independence.
Anchored in Belonging
Our overarching belief is that every young person deserves a place to belong. Guided by that belief, we developed our long-term strategy to end youth and young adult homelessness in the Pikes Peak region—called HOME.
HOME is:
Housing that is safe, stable, and affordable
On-going positive relationships with safe adults and peers
Meaningful engagement in education and employment
Emotional, social, physical, and psychological well-being
Case management and wraparound services are how we put HOME into practice. Each youth’s individualized plan connects directly to these four pillars, ensuring that every step forward supports lasting stability and belonging.
What Wraparound Means
“Wraparound” means that no young person has to navigate challenges alone. Each youth is paired with a dedicated case manager who coordinates care across housing, health, education, employment, and community connections. Together, we identify strengths, set goals, and take steps forward—adjusting along the way as needs change.
Services We Provide
Through case management and wraparound supports, young people can access:
Housing & Stability: Safe housing pathways, landlord navigation, tenancy coaching, and move-in support.
Education & Employment: High school re-engagement, job readiness, training, and career pathways.
Health & Well-Being: Connections to medical and mental health care, substance use supports, and insurance navigation.
Life Skills & Documentation: Help obtaining IDs, building financial skills, learning everyday essentials like cooking and transportation.
Legal & Advocacy: Support with court obligations, victim services, and system navigation.
Community & Belonging: Access to mentors, positive peer groups, and safe spaces to feel connected.
